Fill in the official registration form below, then save or print your completed copy. Bring the printed registration form and a table display form for each model to ScaleFest.

Contest forms are not submitted online or in advance. Contestant numbers are assigned by registration staff at the show.

If your phone or browser only previews the PDF, open or download it and use a PDF reader such as Adobe Acrobat to fill, save, and print it. Use the PDF viewer’s download/save control to retain your completed entries.
