Contest entry form

Fill in your contact details and model list below, then save or print your completed copy. Bring all printed contest forms to the show; nothing is submitted online or in advance. Contestant numbers are assigned by registration staff.

Use the PDF viewer’s save/download or print control when finished. On phones or browsers that only preview PDFs, use the open/download link and a PDF reader such as Adobe Acrobat.

Table display form

Complete one table display form for each model entered. Include the model description, kit maker, techniques, and applicable award information.

Print and bring a copy for each model. Place the form under or near your model with the modeler’s name facing down, as directed on the form. No advance submission is required.

Vendor application

The vendor application and final vendor details will be available closer to the event. For questions about participating, contact the chapter.
